The wife of the wounded Stanislav Ivanovich Pistochenko, born in 1974, commander of the rifle division of the Armed Forces, turned to the “Healthy Bones Restorative Orthopedics Center”. In October 2022, during the fighting near Stepnogorsk, Zaporizhzhia Region, he received a combined explosive shrapnel wound to his face, chest, abdomen, upper and lower limbs. At the stages of treatment, Stanislav underwent a series of surgical interventions in the abdominal cavity, facial bones, and lower limbs. Currently, Stanislav is being treated in the trauma department of the emergency medical care hospital in the city of Zaporizhia.
Stanislav’s wife appealed because her husband was asked to perform an amputation of the left lower limb at the level of the thigh.
On the basis of the x-rays and photographs of the left leg of the wounded man presented by his wife, it was determined that Stanislav has the consequences of a severe explosive shrapnel injury to the left leg. Inflammatory osteomyelitis of the left tibia, a defect of the left tibia in the upper third for up to 7 cm, a consolidating fracture of the left tibia and fibula in the lower third, the condition after extra-focal osteosynthesis of the left tibia with a spoke-rod type apparatus, a scar-ulcerative defect of the soft tissues of the upper third of the left leg, granulation wounds of the upper third of the left leg, reduction of the left leg to 5-6 cm.
Yes, Stanislav’s situation is difficult, but not hopeless. It is too early to talk about amputation. The limb can be preserved, but this will require a series of staged reconstructive and restorative operations using Ilizarov technology.
During the first stage of surgical treatment, it is planned to perform the dismantling of the external fixation device, segmental resection of the necrotic area of the tibia, extrafocal transosseous osteosynthesis of the tibia with the device of the tibia and femur with fixation of the knee joint with the elimination of all existing deformations with a skin flap. Two weeks after the operation, the dosed movement of the bone fragment in the apparatus will begin to replace the tibial defect.
The cost of the first stage of treatment is UAH 191,650 (approximately USD 5,600 or EUR).
At the second stage of surgical treatment (approximately 3 months after the first stage), an open alignment of bone fragments of the tibia will be performed.
The cost of the second stage of treatment is UAH 69,615 (approximately USD 2,050 or EUR).
During the third stage (approximately 4 months after the second stage), it is planned to dismantle the femoral segment of the external fixation device, remount the external fixation device in the lower third of the lower leg, and perform a semi-closed osteotomy of the tibia and fibula. Two weeks after the operation, dosed traction in the apparatus will be started in order to form traction bone regenerate and restore the length of the lower leg.
The cost of the third stage of treatment is UAH 94,690 (approximately USD 2,800 or EUR).
