Serhii Petrovych Lapiichuk, born in 1995, a foreman in military unit A 7170.

During the hostilities on July 5, 2022, he received a landmine wound on his left foot with damage to the heel area. He was evacuated to the Medical Center of the SR, where he underwent primary surgical treatment of the heel area, and Serhiy was evacuated for further treatment to the Lutsk City Hospital, where he was repeatedly subjected to necroctomies and VAK-therapy of the wound. Given Serhiy’s presence of a heel bone defect, he was offered an amputation, which he refused.

From September 11 to November 15, 2022, Serhiy with a diagnosis of “Mine-like injury of the left lower limb. The state after multiple necroctomies and VAK therapy. “Defect of soft tissues and heel bone” was treated in the department of microvascular, plastic and restorative surgery of the National Institute of Surgery and Transplantology named after A.A. Shalimov, where he underwent plastic surgery of a soft tissue defect by transposition of a sural flap.

At the time of the examination on November 14, 2022, Serhiy has: “Consequences of a landmine wound on the left foot. The condition after plasty of the soft tissue defect of the heel area with a sural flap. The flap has taken root. Subtotal defect of the left calcaneus (absence of the body of the calcaneus). Achilles tendon defect. Violation of the function of plantar flexion of the foot, violation of the function of walking.”

To restore the walking function of the left lower limb, Serhiy is shown performing staged reconstructive and restorative surgical treatment using regenerative medicine technologies: autologous mesenchymal stem cells, periosteal cells, and 3D cell carrier printing technologies.

At the first (preparatory) stage, it is planned to collect red bone marrow and a fragment of the fibula bone, manufacture a 3D model of the heel bone and a spacer model of the heel bone defect, and then a 3D model of the cell carrier (transplant). Mesenchymal stem cells and periosteal cells will be isolated from the obtained biomaterial. These cells will be cultivated to the required number and will be used in the future to create osteoprogenitor grafts to replace the defect of the heel bone.

The cost of the first stage of treatment is UAH 247,825. (approx. USD 6900 or EUR).

At the second stage of surgical treatment (approximately 3 weeks after the first stage), it is planned to perform extra-focal osteosynthesis with a shin-foot apparatus, to bring the foot into plantar flexion and to start preparing the skin flap for the next operation.

The cost of the second stage of treatment is UAH 51,819 (approximately USD 1,530 or EUR).

During the third stage (approximately 5-6 days after the second stage), it is planned to perform a revision of the flap with the formation of the recipient bed for further placement of the osteoprogenitor graft of the calcaneus defect.

The cost of the third stage of treatment is UAH 28,865 (approximately USD 850 or EUR).

At the fourth stage of treatment (4-6 weeks after the third tap), the external fixation apparatus will be dismantled, the osteoprogenitor graft of the calcaneal bone defect will be placed in the recipient bed and fixed to the remaining fragment of the calcaneal bone.

The cost of the fourth stage of treatment is UAH 78,453 (approximately USD 2,290 or EUR).

After restoring the integrity of Serhiy’s heel bone, it is planned to restore the defect of the Achilles tendon and the plantar flexion function of the foot. Taking into account the inflationary processes taking place in the country in connection with the war, the calculation of the cost of the stages of treatment for the restoration of the right heel bone will be carried out after the completion of the fifth stage of treatment.
