On July 13, 2022, during hostilities in the Mykolaivka settlement of the Donetsk region, he received an explosive shrapnel wound to the left leg with a soft tissue defect of the left leg and a gunshot defect to the tibia and fibula bones, and a wound to the right foot with a fracture of the fibula. After providing first aid, he was taken to the Dnipro City Clinical Hospital, where the left lower leg was fixed with a rod-type device, and the right foot was fixed with a plaster cast. From 07/19/2022 to 08/03/2022 he was being treated at the National Cancer Institute.
Since August 4, 2022, he has been receiving treatment at the department of microvascular, plastic and restorative surgery of the National Institute of Surgery and Transplantology named after A.A. Shalimov, where he underwent surgical treatment with the replacement of soft tissue defects on the front-outer surface of the left lower leg.
At the time of the examination on 10/14/2022, the patient has: “Consequences of a severe explosive shrapnel combat wound to the left lower leg. Inflammatory defects of the left tibia and fibula with a bone tissue defect up to 12 cm long. The condition after plasticity of soft tissue defects of the front-outer surface of the left leg with skin-fascial-muscle flaps on the feeding artery. The condition after extra-focal osteosynthesis of the left tibia with rod-type devices. Instability of the external fixation apparatus”.
Oleg’s presence of critical defects of the tibia and fibula bones of the left lower leg creates significant problems during treatment. The use of classical treatment methods to restore the strength of the left lower limbs will be ineffective and will be associated with a significant risk of unsatisfactory treatment results and possibly amputation of the left lower leg.
In order to restore the function of the left lower leg as much as possible and minimize the risks of its amputation, the use of bioregenerative technologies is shown during Oleg’s treatment. Integrated use of Ilizarov’s regenerative technology, cell-tissue technologies and 3D printing technology is planned. Reconstructive and restorative surgical treatment is planned to be carried out in stages.
At the first stage of operative treatment, it is planned to perform a revision of the area of the bone defect of the left fibula, extra-focal transosseous osteosynthesis according to Ilizarov of the left tibia, osteotomy of the left tibia and fibula in the upper third, followed by dosing.
The cost of the first stage of treatment is UAH 90,252 (approximately USD 2,510 or EUR 2,580).
At the second stage of surgical treatment (approximately 4 months after the first stage), perform an open alignment of the bone fragments of the tibia and remount the device in order to continue the traction of the fragment of the tibia.
The cost of the second stage of treatment is UAH 28,842 (approximately USD 805 or EUR 825).
During the third stage (biotechnological stage) of treatment, it is planned to carry out osteosynthesis of the fibula bone with a plate, the collection of red bone marrow and a fragment of the fibula bone for the isolation and cultivation of mesenchymal stem cells and periosteal cells. These cells will later be used to create an osteoprogenitor graft to replace the tibial defect. Also during this period, a spacer for the left tibial bone defect will be created using 3D printing technology.
The estimated cost of the third stage of treatment is UAH 308,234 (approximately USD 8,570 or EUR 8,810). Due to the fact that the implementation of the third stage of treatment will take place already in 2023, the cost of cellular material cannot be determined, because the medical company Good Cells plans to review the cost of cultivating mesenchymal stem cells and periosteal cells.
The fourth stage of treatment will be carried out after the traction bone regenerate of the tibia is completely strengthened. During this stage, complete reassembly of the external fixation device and formation of the recipient bed will be performed.
The estimated cost of the fourth stage of treatment is UAH 31,625 (approximately USD 880 or EUR 910).
At the fifth stage of treatment (approximately 4-6 weeks after the fourth stage), it is planned to use 3D printing technology to create a cell carrier based on the polymer material “trabeculoplast”, to manufacture an osteoprogenitor graft and to perform its transplantation into the tibial bone defect.
The estimated cost of the fifth stage of treatment is UAH 118,680 (approximately USD 3,300 or EUR 3,400).
After the formation of new bone tissue in the area of the tibial bone defect, it is planned to dismantle the external fixation device.
