Patient Dmytro Serhiyevich Ozhelevskyi, born in 1991, inspector, private assault company of the Dnipro-1 police regiment, approached us with the question of the possibility of lengthening the stump of the left lower leg, because they cannot make a lower leg prosthesis for him and they suggest amputation above the knee.

During a combat mission on July 26, 2022, in the area of Dmytro, a resident of Sumy Oblast, received a combined explosive shrapnel wound with a shrapnel wound to the abdomen, multiple shrapnel wounds to the limbs with a fracture of the bones of the left leg and crushing of the soft tissues of the left leg. He underwent an emergency operation – the left lower leg was amputated at the level of the middle third. Then, at the stages of Dmytro’s treatment, reamputation of the stump of the left leg was performed, as a result of which a short stump was formed.

 

To solve the issue of prosthetics, Dmytro was sent to the rehabilitation center of the Netherlands in the city of Utrecht. In the Netherlands, he was offered an above-the-knee reamputation, because the stump of his leg is very short (5 cm), and for prosthetics it is necessary that the stump be at least 10 cm.

Dmytro applied to various medical institutions, both in Europe and in Ukraine, but everywhere they refused him, referring to the fact that it is impossible to lengthen the stump, and that amputation should be done above the knee. But it is not so. Technology G.A. Ilizarov allows you to lengthen the existing bone tissue of the stump to the required size. The use of this technology was suggested to Dmytro.

Dmytro’s surgery treatment will take place in 2 stages.

During the first stage, an extra-focal transosseous osteosynthesis will be performed with a femoral-tibia device and a bone-plastic osteotomy of the left tibial stump will be performed.

The cost of the first stage of treatment is UAH 87,285 (approximately USD 2,430 or EUR 2,500).

The second stage of surgical treatment is planned approximately 5-6 months after the traction bone regenerate strengthens.

Taking into account the inflationary processes taking place in the country in connection with the war, the calculation of the cost of the second stage of treatment will be carried out about a month before the planned time of the stage.

After lengthening the stump of the left lower leg, prosthetists will be able to make Dmytro a full prosthesis, and Dmytro will begin the rehabilitation process.
