The patient Maksym Leonov, born in 2001, a soldier in military unit A2585, turned to our Foundation.
During the hostilities on August 7, 2022, near the village of Pisky, Donetsk region, he received a landmine wound to his left foot with the separation of the front part of the foot. He was initially treated at hospital No. 66 in Pokrovsk. Then he underwent treatment at the Third Cherkasy Emergency Hospital, where he was offered an amputation at the level of the lower leg, which Maksym refused. Further treatment took place in the department of microvascular, plastic and restorative surgery of the National Institute of Surgery and Transplantology named after A.A. Shalimov, where he underwent plastic surgery of the back of the stump of the left foot with a thoraco-dorsal flap.
At the time of examination on February 3, 2023, Maksym has a stump of the left foot at the level of the Schopar joint with a complete violation of the support function. X-ray examination of the stump of Maksym’s left foot showed signs characteristic of necrosis of the talus bone. An MRI examination confirmed these suspicions.
After the examination, Maksym was given the following diagnosis: “Consequences of a combat explosive wound to the left foot. Stump of the left foot at the level of the Chopar joint, avascular necrosis of the talus bone, equinus installation of the heel, the condition after the plastic defect of the soft tissues of the left foot with an innervated thoraco-dorsal flap, pronounced violations of the support function of the left foot.”
Unfortunately, it is impossible to restore the necrotic bone tissue of the talus according to our technology using stem cells and vascularization of the talus.
In this situation, in order to restore the function of support and the left foot as much as possible, it is necessary to restore the second point of support of the foot, which is located in the area of the base of the fifth metatarsal bone. The third point of support, which is located in the area of the head of the first metatarsal bone, will be compensated by a special insole-prosthesis. The second point of support can be restored by staged surgical treatment using the technologies of regenerative medicine: autologous mesenchymal stem cells, periosteal cells and 3D printing technologies of the cell carrier.
At the first stage, it is planned to perform an astrogalectomy (removal of the talus) with the formation of a tibiocalcaneal arthrodesis and to perform a tibial osteotomy in order to restore the length of the left lower limb after the removal of the talus.
The cost of the first stage of treatment is UAH 153,610 (approximately USD 4,480 or EUR).
During the second stage (approximately 4-5 months after the first one), it is planned to collect red bone marrow and a fragment of the fibula bone, make a 3D model of the metatarsal bones, and a model of the spacer of the metatarsal bones, and then a 3D model of the cell carrier (transplant). Mesenchymal stem cells and periosteal cells will be isolated from the obtained biomaterial. These cells will be cultivated to the required number and will be used to create an osteoprogenitor graft.
The cost of the second stage of treatment is UAH 338,945. (approx. USD 9900 or EUR).
At the third stage of surgical treatment (approximately 5-6 weeks after the second stage), it is planned to perform a flap revision with the formation of a recipient bed in the end of the stump of the left foot and the formation of a recipient bed on the anterior abdominal wall. In the future, an osteoprogenitor graft of the metatarsal bones will be placed in the area of the bed on the front abdominal wall for its preforming.
The cost of the third stage of treatment is UAH 82,690 (approximately USD 2,450 or EUR).
At the fourth stage of treatment (4-5 weeks after the third stage), the osteoprogenitor graft will be placed in the bed on the anterior abdominal wall for its preforming.
The cost of the fourth stage of treatment is UAH 57,140 (approximately USD 1,670 or EUR).
During the fifth stage of treatment (6-8 weeks after the fourth stage), extraction of the osteoprogenitor graft from the bed on the front abdominal wall of the abdomen and its transplantation into the area of the end of the stump of the left foot will be performed.
The cost of the fifth stage of treatment is UAH 107,555 (approximately USD 3,140 or EUR).
