Bohdan Viktorovych Mulyarchuk, born in 1992, senior soldier of military unit A 4053.
During the hostilities in the city of Lysychansk, Luhansk region, on June 20, 2022, he received gunshot and explosive shrapnel wounds to the heel area of the right foot and right leg with soft tissue defects and defects of the right calcaneus and left tibial bones.
The first medical aid was provided in the city of Kramatorsk, where he underwent primary surgical treatment of wounds and fixation of the limbs with rod-type devices, after which he was evacuated to hospital No. 6 in the city of Dnipro. Then, on June 26, Bohdan was evacuated to Kyiv to city hospital No. 1, where he underwent repeated surgical treatment of wounds and VAK therapy of wounds. Since July 11, Bohdan has been receiving treatment at the Department of Microvascular, Plastic and Reconstructive Surgery of the National Institute of Surgery and Transplantology named after A.A. Shalimova, where he underwent plastic surgery to replace soft tissue defects in the right heel area and the front-outer surface of the left lower leg.
During the examination on 09/23/2022, the patient has: “Consequences of severe explosive shrapnel combat injury to the area of the right heel and left lower leg. Fiery defect of the right calcaneus, multifragmentary fracture of the left tibia with a bone tissue defect up to 12 cm long. The condition after plasticity of the soft tissue defects of the right heel area and the front-outer surface of the left leg with skin-fascial-muscular flaps on the feeding artery. The condition after extra-focal osteosynthesis of the right ankle joint and left leg with rod-type devices.”
Bohdan’s presence of critical defects of the right calcaneus and left tibia, the use of classical treatment methods to restore the bearing capacity of the lower limbs will be ineffective and will be associated with a high risk of unsatisfactory treatment results and, possibly, amputation of the right lower leg.
In order to restore the reproductive capacity of the lower limbs, to restore the function of the damaged limbs as much as possible and to minimize the risks of amputation of the right lower leg, Bohdan is shown performing staged reconstructive and restorative surgical treatment using autologous mesenchymal stem cells, periosteum cells and cell carrier 3D printing technologies.
First of all, it is planned to restore the bearing capacity of Bohdan’s left lower limb in order to activate it and allow him to move with the help of crutches, after which he will deal with the restoration of the defect of the right heel bone.
At the first stage of surgical treatment, it is planned to carry out a revision of the area of the bone defect of the left tibia, extra-focal transosseous osteosynthesis according to Ilizarov of the left tibia, its osteotomy in the lower third with further dosed movement of the bone fragment into the area of the defect.
The cost of the first stage of treatment is UAH 113,482 (approximately USD 3,160 or EUR 3,250).
At the second stage of surgical treatment (approximately 4 months after the first stage), perform a double osteotomy of the fibula to form a tibiofibular synostosis.
The cost of the second stage of treatment is UAH 19,228 (approximately USD 540 or EUR 560).
During the third stage (biotechnological, approximately 3-4 weeks after the second stage), it is planned to collect the red bone marrow and a fragment of the fibula bone. Mesenchymal stem cells and periosteal cells will be isolated from the obtained biomaterial. These cells will be cultivated to the required number and will be used in the future to create osteoprogenitor grafts to replace the tibial bone defect. Also during this period, a spacer for the left tibial bone defect will be created using 3D printing technology. Also, during this stage, it is planned to fix the fibula to the tibia to create a tibio-fibular synostosis.
The cost of the third stage of treatment is UAH 309,675 (approximately USD 8,610 or EUR 8,850).
The fourth stage of treatment will be carried out after the traction bone regenerate of the tibia is completely strengthened and the tibio-fibular synostosis is formed. During this stage, the device will be dismantled and the recipient bed will be formed.
The cost of the fourth stage of treatment is UAH 31,050 (approximately USD 865 or EUR 890).
At the fifth stage of treatment, it is planned to use 3D printing technology to create a cell carrier based on the polymer material “trabeculoplast”, to manufacture an osteoprogenitor graft and perform its transplantation into the defect of the tibia.
The cost of the fifth stage of treatment is UAH 118,680 (approximately USD 3,300 or EUR 3,400).
After restoring the bearing capacity of the left lower extremity, it is planned to begin preparation for the restoration of the right heel bone. Taking into account the inflationary processes taking place in the country in connection with the war, the calculation of the cost of the stages of treatment for the restoration of the right heel bone will be carried out after the completion of the fifth stage of treatment.
